Paper application forms

Applying for Reception, Y5 or Y7 September 2022 on a Paper Application Form

The admissions process for September 2022 opened on the 9th September 2021. 

We strongly advise that you apply online.  If you are unable to, and want to apply on a paper form, the forms are available below to print out.  Please note that if you are applying for a Y5 Middle School place or a Y7 High School place or a Y9 Monkseaton High or Whitley Bay High School place the closing date was the 31st October, any application you make now on a paper form will be classed as 'late' and wont be processed until after National Offer day, the 1st March 2022

Before you apply, please read the information provided

You must return the Reception class form by the closing date of the 14th January 2022.

We advise returning your form to us by email.  If you post it, you must get proof of postage that you can provide if we don't receive your form before the closing date.  Please do not ask schools to return the form for you, you should return it to us yourself.
