Transitioning from children's to adult social care services

Transition is what we mean when a young person moves from being supported by children’s social care services to adult social care services. For example, a young person that has a social worker in children’s services and will need a social worker when they become an adult (at the age of 18).
We want to make sure that that every young person with social care needs (according to the 2014 Care Act), and their family, has the best possible transition between services. So we would like to learn from your experiences and collect your suggestions to help us improve. We are asking young people who are currently transitioning between children's and adult social care services, or who have completed this transition within the last 12 months (and parents and carers) to complete this short anonymous questionnaire.
We will use your insights to help shape new transition pathways that will benefit all of the young people we work with.
